Family Support Coordination Program

The Family Support Program provides a range of individualized supports to families and to individuals in Leeds and Grenville Counties who have an intellectual disability. These include:
•  Information and Referral
•  In-Home support
•  Family Relief/Respite
•  Assistance with participation in valued roles in community life
   (e.g. leisure, friendship, employment, education, etc.)
•  Assistance with applications to government for funds or special assistance.
•  Coordination of services.
•  Planning for future.

Programs provided by the Ministry of Community and Social Services and the Ministry of Youth and Children's Services

People who have an intellectual disability are eligible for funding from the Ontario Government. Each program has its own requirements and benefits

Programs for Children
•  Assistive Devices Program (ADP)
•  Assistance for Children with Severe Disabilities (ACSD)
•  Enhance Respite Funding
•  Out of Home Respite
•  Respite for Children and Youth who are on the Autism Spectrum Disorder.
•  Special Services at Home (SSAH) for children

Passport Funding (New: Includes SSAH for Adults)
